Wegovy is injected once a week, but which dose applies depends entirely on where in the treatment you are. The dose escalation is carefully designed to give the body time to adapt to semaglutide — the active ingredient in Wegovy. If you skip the steps or increase the dose too quickly, the risk of nausea, vomiting and other gastrointestinal problems increases significantly.

The complete schedule spans five dose levels over sixteen weeks, before reaching the maintenance dose of 2.4 mg. Below we go through each step, how the number of clicks is converted into a dose, what applies in the event of a missed injection and which side effects are most common at each dose level.

Wegovy dosing schedule: five steps up to the maintenance dose

Escalation is not optional — it is part of the treatment protocol and exists for medical reasons. The body needs adaptation time to tolerate semaglutide in higher concentrations.

The schedule looks like this:

Each dose level is maintained for at least four weeks. If the side effects are severe during one step, the doctor may extend the period on the lower dose for another four weeks before the next escalation. So it’s possible for the entire ramp-up phase to take longer than the basic sixteen weeks — and that’s perfectly normal.

The effect on weight loss is strongest at the maintenance dose of 2.4 mg, but already at 1.0 mg most people notice a clear reduction in appetite. The starting dose of 0.25 mg is not intended to cause weight loss in itself — its purpose is only to get the body used to the drug.

Wegovy dosing clicks: how to count correctly

Wegovy is supplied in pre-filled injection pens (FlexTouch), and each pen is calibrated for a specific dose. You don’t push down a plunger manually — you turn to the right dose and keep the button pressed until the counter shows 0.

The click system in the pen works so that one click corresponds to a specific volume of the solution. The important thing to understand is that you don’t count clicks manually — the pen is preset for the exact dose to be injected. Each pen is also colour-coded:

  • 0.25 mg — red pencil (starting dose)

  • 0.5 mg — brown pencil

  • 1.0 mg — dark blue pen

  • 1.7 mg — light blue pen

  • 2.4 mg — green pencil (maintenance dose)

Before the injection, check that the solution in the pen is clear and colorless — or slightly yellow — and that no particles are visible. If the solution is cloudy or contains visible particles, the pen should not be used.

The injection is given subcutaneously, that is, in the subcutaneous fat. Suitable injection sites are the abdomen (at least 5 cm from the navel), the front of the thigh or the outside of the upper arm. The injection site should be rotated from week to week to avoid local skin reactions such as redness and hardening of the tissue.

Wegovy starting dose 0.25 mg: what happens in the first four weeks

During the first four weeks of 0.25 mg, many people find that the side effects are mild or non-existent. It is the body’s adaptation phase and the appetite suppression is not yet fully activated. Nausea may occur, but is rarely intense at this dose level.

Some patients are surprised that they don’t notice a clear weight loss right away — but that, as I said, is not the purpose of the starting dose. Weight reduction generally accelerates when the dose increases towards 1.0 mg and above. Not skipping the starting dose, even if it seems “weak”, is central to avoiding problems later in the treatment.

Dosing wegovy: side effects per dose step

The side effect profile changes with the dose. At lower levels, gastrointestinal distress of a mild nature predominates, while more noticeable symptoms may occur upon escalation to 1.7 mg and 2.4 mg.

Side effects to expect at each dose level

0.25 mg (weeks 1–4): Mild nausea after the injection, possible loss of appetite, sometimes fatigue. Most people manage this phase without major problems.

0.5 mg (weeks 5–8): The nausea may intensify slightly, especially in the days immediately after the injection. Constipation or loose stomach are common. Vomiting occurs but is uncommon.

1.0 mg (weeks 9–12): Appetite suppression is now evident. The nausea may persist but usually decreases in intensity as the body gets used to it. Heartburn and acid regurgitation are added for some patients.

1.7 mg (weeks 13–16): Escalation step with the highest risk of side effects. Nausea, vomiting and diarrhea are more frequent. Pancreatitis is a rare but serious side effect that requires immediate medical attention for symptoms such as severe abdominal pain radiating up to the back.

2.4 mg (maintenance dose): The side effect profile is similar to 1.7 mg but tends to stabilize over time. Studies show that up to 44% of patients report nausea during the ramp-up phase, but this rate drops to around 15-20% at steady state on the maintenance dose.

General advice to reduce side effects: eat slowly and avoid fatty or strong meals, drink enough water, avoid lying down immediately after eating. Alcohol in combination with Wegovy often increases the nausea.

Wegovy escalation: what happens if you miss a dose

Missing an injection happens — but what counts depends on how long it’s been since the missed dose.

If it has been five days or less since the scheduled injection: take the missed dose as soon as you remember. Then move your regular weekly day of injections to the new day, or return to the original day the following week — at least five days must pass between two injections.

If it has been more than five days: skip the missed dose and take the next dose on your regular day. Never take a double dose to compensate.

In the event of a longer break — for example, if you have paused treatment for more than two weeks for practical or medical reasons — you should contact your prescribing doctor. In some cases, a re-escalation may be necessary, especially if you increased the dose and then stopped treatment. Going back to a lower dose and increasing again reduces the risk of side effects when restarting.

There is no clinical data to support that one can “make up” lost weeks by speeding up the escalation. Each step requires its four weeks — it is not negotiable from a security perspective.

Wegovy 2.4 mg: the maintenance dose and what happens if it is not tolerated

Wegovy 2.4 mg is the dose where the clinical studies (STEP program) have documented the strongest weight reduction. In the STEP 1 study, participants who fully tolerated the maintenance dose achieved an average weight loss of just over 15% of body weight after 68 weeks — compared to around 2.5% in the placebo group.

But not everyone tolerates 2.4 mg. If the side effects on the maintenance dose are unacceptable, there is the option of reducing to 1.7 mg permanently. Even 1.7 mg produces clinically significant weight reduction, albeit lower on average than with 2.4 mg. The decision to remain on a lower maintenance dose is always made in dialogue with the prescribing physician.

If treatment is stopped completely, weight usually returns gradually. The STEP 4 study showed that patients who stopped Wegovy after 20 weeks had regained about two-thirds of their lost weight at one year. It emphasizes that Wegovy is a long-term treatment option rather than a cure with a fixed end date.

Another situation that requires attention is if you switch from another GLP-1 agonist — for example, Ozempic — to Wegovy. Although both contain semaglutide, the dose levels and escalation schedules are not identical. The change must take place under medical supervision and requires a clear plan for which dose level to start at in the new schedule.

Having a realistic view of what Wegovy dosing actually entails—sixteen-week ramp-up, weekly administration, side-effect management, and a lifelong perspective on treatment—is essential for treatment to produce the results the clinical data promise.
